New Patient Registration
The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
You may print off a registration form & medical questionnaire , fill it out and either post it to the surgery or bring them into the practice.
Please allow a week for the registration to be completed
Your form will be triaged by our Nurse Practitioner who will then advise ypu in writing if you need to come in for a new patient check
Please note if when we receive your application that you are outside of our area we will inform you that we will be unable to accept your registration.
When you send in your form please dont forget to enclose a copy of your last repeat prescription script if you are currently taking repeat medication
